The Client

• Someone in need of vital medical/rehabilitation equipment who does not have the financial means or insurance coverage to obtain it elsewhere.
• They contact or are referred to Ability Found to see if we can help.
• Ability Found establishes a relationship with a professional service provider in their area to help recommend the most appropriate and beneficial type of equipment

The Client's Responsibilities

• Provides the registered service provider with accurate personal and medical information for the completion of the Equipment Evaluation and Request Form
• Upon approval uses the purchased equipment according to manufacturer's specifications and as instructed by the professional service provider and medical equipment vendor.
• Participates with the medical equipment vendor to submit a photo of the equipment in use with a letter.
