Professional Service Providers

The Registered Service Provider

• A licensed professional such as a physical therapist, occupational therapist, and speech language pathologist, who has registered with Ability Found.
• Must have experience and expertise with the equipment in which they are recommending.

Registered Service Provider's Responsibilities

• Gathers information from the client to complete the Equipment Evaluation and Request Form.
• Consults with a Registered Medical Equipment Vendor and Ability Found to help determine the most appropriate piece of equipment
• Submits the completed Equipment Evaluation and Request Form to Ability Found for review.
• Assists medical equipment vendor with fitting of equipment and training client in its use.
• Monitors the use of the purchased equipment while the client is under their care.
